Scouting sheets: Willing to share what you have?

Hey all,
We've always been graciously thankful at asking and borrowing other team's scouting sheets during past competitions. Somehow, I could never get our students to create one that I was completely happy with.
Is there anyone willing to post a link to one that they are willing to share?

I can email you a copy of the one I'm playing with right now in Publisher... however I have to warn you it is very much a work in progress. This is my first year creating it in Publisher instead of Word and so far its working better. More control over text-boxes. Anyone who PM/Emails me can get the current version.
