Does Anyone Have a Liability Form?
 
As the title implies, I'm looking to see if any team has a travel liability form that our team could modify and send out to the students before Kickoff. I tend to have a bit of difficulty downloading things from the White Papers, so if at all possible, I would ask whomever can help me to send it via email to cutiebethy@yahoo.com. Thank you!

You might try looking here http://lawyer.com/ and search for 'Liability Waiver' They have a huge list of different waiver forms - one will probably be easily modified to fit your needs.

It all depends on what liability you need. If this is for kids to work at a school or for mentors to work with kids you will need to talk to the school. You will need to find out what they want the form to say. The bottom line is to make sure you keep the right people happy.

I can send you ours, it doesn't say much on purpose.

Mostly what I need is a form that will release us from travel liability. We won't be able to take a school bus to the Kickoff Event, so the team's mentors (a few college kids) will be driving the students to the event. Basically what I need is a form that says that if something happens at the event or during the travel time that the kids can't turn around and sue us. If anyone has a form of this sort, I would very much appreciate it.

IANAL, but I believe that the field trip forms from District Five had the following boilerplate after asking for emergency numbers:

Our school board has a bunch of various forms that we have to fill out as clubs for liability. Our team is very tough when it comes to liability issues. As part of a school, you can find out what forms are truely necessary to go places. And it's not that hard to make one really. Emergency contact, insurance is a key thing and signuatures. Some of our forms require a notary.
