How do I send the Paperwork for AVA?

I cant figure out how to upload the AVA paper work. :ahh: I don't know how to save only the pages 18 -29 in a different file. And how can I type in It. Or am i doing this the wrong way. What is the best way to fill out the paperwork and send it to the website?

You can get the .doc format paperwork off of Autodesk's Streamline site, don't try to use the PDF.
