For all of you coaches out there - does anyone have a comprehensive medical/insurance form that you make the kids fill out for travelling/competitions? We don't have an official one here at the school, and I was trying to put one together, and didn't totally know what info to ask for. If someone could e-mail one to me, or attach it, I would be forever in debt to you.

Caution to all coaches. Make sure that ALL students have medical coverage and you are carrying proof of it. We have made several trips to the hospital and it is very expensive as they ask for proof or cash up front. We know by experience. If we have a student with no coverage, our school district makes arrangements for a short term policy effective for that trip only.
