Ordering Information:
1. All Pre-Order Forms MUST be received no later than March 4, 2011.
2. Full payment is due at the time of order.
3. Orders may be emailed or faxed
4. All orders will be confirmed by email or phone and will include pickup time and
location.
5. Cancellations are due by Monday, March 4, 2011 by noon. Cancellations after March 4, 2011 will be charged 100%.
6. To ensure easy processing, please assign one team point of contact.

Please choose from the following Deli Lunch Box options. All lunches include a sandwich on a
freshly baked croissant or roll, Teddy Graham cookies, chips and choice of beverage.

Deli Lunch Boxes: $8.50
Ham – Delicious ham and Swiss cheese served on a French roll with lettuce, tomato & pickle.
Turkey – Tender turkey breast and Cheddar cheese served on a croissant with lettuce, tomato &
pickle.
Chicken Salad – Lean and flavorful chicken salad served on a croissant with lettuce,
tomato & pickle.
Vegetarian – Sliced seasonal veggies and portobello on a French roll with lettuce, tomato, and pickle.
