1 - Is the analog module in the slot closes to the cRIO power connector?
2 - Is the jumper on the breakout board installed? (it is the outer 2 pins)
3 - Is there a wire to running from analog breakout board to the PDB?
4 - Did you put a fuse in the PDB?
If it still does not work...
5 - Do any other channels on the analog module work? Perhaps the breakout module is kaput?
6 - Can you check the voltages on the breakout board? Is the "Power Good" LED on the breakout board lit up? Perhaps the breakout board is broken. It has some active circuitry on it.

The module is closest to the power connector, the jumper is on the outer two pins, we do have a wire running from the breakout board to the PDB, and we do have a fuse.
The analog module works, we tested it, and we checked the voltages.
