MPLAB is Microchip's IDE that supports assembly language programming, only, by default. C18 is a C compiler for the PIC18 series microcontrollers. C18 acts as an addon to MPLAB.

MPLAB is free, C18 is not.

Given compiler compatibility, I'm pretty sure you can use both the version from IFI's cdrom or from Microchip's website, interchangeably. The thing about the IFI cdrom is that it contains a completely unlocked version of C18, rather than a 60-day trial version. Microchip wouldn't like it if IFI had a completely unlocked copy of C18 available for download on their website.
