There were 4 items in the LabVIEW package that my team received:
1. A shrink wrapped "CD Kit" containing software installation CDs.
2. A "Getting Started With LabVIEW" pamphlet.
3. A one page "Licence Agreement".
4. A one page (folded) "Certificate Of Ownership" with CD stickers and perforated registration cards.
The latter item is what you are looking for...
