I received this from Autodesk, Inc and they said I could post it here:

We have not posted the entry form yet. I will send you an email when it is posted.

You do not need to fill out the entry form ahead of time like last year. You will complete this web based form once you are ready to submit your animation. Are you ready to submit your animation entry? The form will be web based and we will not be using Streamline this year.
