We've got reCAPTCHA, e-mail verification, a simple question (2+2=, etc) and I've banned anonymous proxies and a few free/problematic e-mail domains.

And we use Akismet to filter all posts until members prove they aren't spam bots. This last feature has cut down on the amount of spam dramatically, as far as i can tell.
