Also, when they fail, has anyone done a post-mortem to see how - i.e. gearbox failure, armature burned out, bearing failure, et cetera.

Ben,
Properly designed, there should be no problem. We have robots going back over six years that still have their original motors.
In our first and second years, we had problems with gearing and side loads and, yeah, we toasted a few. If your motor gets hot, look at the problem closely. The most likely problem is too much friction or side loading in your system.
Bottom line: We replace motors when they fail.
