I've seen this happen in empty goals, so I think there's something else that can cause it.

Regardless, the cause is the same; the ball counter working the wrong way. Every time I've seen it happen it was corrected (either based on the authority of n people other than myself who caught it, or the authority of a volunteer counting the in/out manually).
