In 2008 we had encoders do out twice on our bot. when this occurred, the robot would run into the far wall at about 15 FPS. This did an amazing amount of damage to our bot as well as knocking off the controls of an opponent once.

Alan is right, typically it is a wire, though with the new control system, there are several different wires that can cause this failure mode.
