Converter / Convertor outputting 12V instead of 5V

We're following the 2011 FRC Power Distribution Diagram - Alternative configuration 24V solenoid. Link:

In it, it tells us to connect the converter to the router. The converter is supposed to output 5V according to the label on its back. However, according to our measurement it is outputting 12V.

Could it be a faulty converter? Can someone measure the voltage being outputted on their converter and post their results?
