2012 Chairman's Video Subject Consent Form?

I've been looking for a video subject consent form online somewhere and I can't seem to find it on usfirst.org.

The 2012 rules mention this part:
Content Guidelines:
o Important Note: If you include full names or if you include any name and the
location of a minor (e.g., a student at Kennedy Elementary School in Bedford
Falls, MA), you will need to supply releases from the minor’s parents.

We want to include FULL NAMES in the video of speakers. Where is this "release" form the person we are interviewing?

We found the consent form for the DVD the "team consent" but it doesn't seem to cover the person we are video taping.

Anyone have any pointers where it might be?
