To COMPLETELY reset the D-Link, do the following:

1. Hold the reset button for 30 seconds while it is still turned on
2. Unplug the D-Link while still holding the reset button. Hold it for another 30 seconds
3. Plug it back in while still holding the reset button.

That should do it.

The default address of the D-Link is 192.168.0.50. You should statically set your IP to something like 192.168.0.51. So:

192.168.0.51
255.255.255.0
192.168.0.50
8.8.8.8

Then run the utility and you should be all set.
