You did verify that the encoder is wired and plugged in correctly, including plugged into the correct ports?

I know its a basic question, but sometimes they need asked.

A (blue) to the signal pin on PWM connectore #1
Gnd (brown) to the ground pin of PWM connector #2
B (yellow) to the signal pin on PWM connector #2

Is what we are doing and we have the two PWM's into the DIO ports on the roboRIO.

Using ports 0,1 aswell and have tried switching, flipping and inverting them.

Ether, can you comment on this? This doesn't seem right to me, shouldn't the ground be shared between both channels?

All of the ground pins should be connected internally.

Yep we get it gives us voltage readings from .4 to 5 v and we are getting a nice voltage feed on the 5v cable and ground so we do have electrical readings.

Sure! What we did was while running an autonomous movement code we were measuring dc voltage across the +5 and the common and we got a reading of +5. Then we ran it across the signal cables to the common and it was showing a square wave pretty much.
