I happen to have a bookmark folder of fails I've stumbled across.

2008:

My favourite match in all of FRC, make sure you have sound on

2011:

469 having their way with 1114 (this is less of a fail and more of 469 dominating)

2012:

649's shooter wheel falls out

2073's turret falls off

610, 1241, and unknown third team attempt to triple balance on the coopertition bridge

469 flying off the bridge

Andrew getting hit in the face

2013:

1918 Falls off the pyramid in Curie finals, hits the bar on the way down.

Unknown team takes out ceiling with frisbee shooter prototype

2014:

1323 manages to score their plunger into the low goal in autonomous

Oh, there's another one I remembered. I'll find a video in a bit. During one of our 2014 offseason competitions, we were about to start a finals match, and as is tradition, were in the process of shaking hands with our opponents. While we crossed the field, 118 randomly fired a ball while the robots were disabled - and they made it.

I remember asking "Is that supposed to happen?"

I've almost gotten to the point where I can watch the start of match 116 on Carson without cringing. That's where we forgot to check our encoders, causing our auto program to do a little 'mountain climbing':

Fortunately we recovered, and it ended up being a decent match for us. For a moment though, I nearly chewed my fingers off.
