Are you setting the motor in any place other than Periodic Task.vi? (Such as in Teleop.vi)

You should only have one setter in any system. Any more is asking for trouble.

Yes the motor is only in Periodic Vi

The Safety neutralizes the PWM outputs if the motor setting isn't adjusted at least every 100ms.

See where the Safety is enabled in Begin.vi
It needs to be either Disabled in Begin.vi (just change the input) or Disabled at the beginning of the sequence and re-enabled as the last step of the sequence.

The same thing holds true for handling motors in Autonomous.
