Similar issues. On some that do go through the consent form does not show. Other things appear to happen randomly. Two months after confirming my Lead Mentor status I got an email yesterday acknowleging this. At about the same time our new Team Administrator's status was confirmed after being in limbo for many weeks.
We've been patient as the FIRST IT staff has had bigger fish to fry lately. I don't think it is user error but it does seem to be getting better... TW
