I was going to ask if it had been dropped. It gives every indication that one of the lenses has become displaced or the the CCD (pickup device) has become dislodged. You didn't indicate what type of camera it is so I can't speak to what to look for. If it is dead already and out of warranty, you could try looking inside. I would start by looking into the lens when the camera is turned on and see if there are any lenses that are moving around when shaken. These should be pretty obvious. If you can view the picture on a screen, then point at something with a lot of light and try moving the lens to see if the picture goes in and out of focus. That would point to the lens being bad.
Try tapping the back of the camera and see if focus changes, that would indicate the CCD is moving. Some CCD have a heatsink attached and that has some weight to it that may have broken a mounting screw during a fall. If that is the case, the repair might be easy. All things being equal, a camera worth a few hundred dollars may not be worth fixing.
