Where to?

The manual says we can upload our storyboard, the entry form, and the consent form all to streamline, but where should we upload the storyboard and what should be the name of it? I'm kind of afraid now since I don't think I have time to send it out to California (The paperwork) since it said all of it could be uploaded electornically.
