I don't think FIRST has a definite form yet. They'll have created one before any of the regionals begin, so you can attach the forms then.

When they do have one, it'll most probably be under the "championship event" link on the FIRST robotics webpage.

A little early to be worrying about that, hm?

hey, i was completely wrong:
apparently, the forms will be mailed (not emailed) to all team contacts on Feb 11th...it's in the last e-mail blast sent today...
