Encoder Wiring Quesiton

OK... I was searching for how to hookup the Agilent quadrature encoder our team is using. I came across this site and searched but my questions weren't completely answered.

Our encoder has 4 wires: Phase A, B, Gnd, +5V. My question is how are they hooked on the RC? I get that the A and B are hooked separately. Are both hooked to the SIG slots? Secondly, are the Gnd and +5V inputs hooked to other Digital I/O pins or a set of the above?

Thanks for any help you can provide.
