One company I worked for only had 6 engineers, but they still wanted us to go through a formal yearly performance review
they passed out forms for us to fill out, detailing our accomplishments for the previous year, and skills that should be taken into consideration for our yearly raise. I could not take it seriously, so I...well, exagerated a little. I wish I had kept a copy, but the one part I do remember is: "Kens engineering and creative skills are second only to God. His contributions to the company cannot be comprehended". Of course, after having fun with it, I wrote the real version of the self-appraisal form and stapled it to the back.
