Vex controller - outputs not working

Hi! This isn't technically about the FIRST Vex Challenge, but it is about Vex. I figured this would be the best place to post it.

Anyway, I'm working on a robot for my science team. However, I've been having some trouble getting the controller to work properly. A summary of the problem is below:

The only PWM outputs working on the Vex controller seem to be ports 2 and 3. Switching from 23 mode into 12 mode doesn't enable output 1. The problem isn't with the motors, since they all work when plugged into 2 or 3. Resetting the control configuration doesn't work. Playing with edit points or trim doesn't work. Changing the batteries doesn't work. Using the tether instead of the radio doesn't work. Using the Rx2 port instead of the Rx1 port doesn't work. The outputs all need to be independent, so setting jumper 15 to turn on 4WD won't solve the problem (I haven't tested it if works at all). I've tried everything that I can think of, but only outputs 2 and 3 work. Any ideas on what I can do to make the other outputs work?
