I can clear up some of the speculations and confusion right now, speaking as a volunteer who was involved in the goings-on at the scoring table.

True, the counting system from National Instruments was not working properly all the time. That's why the data it generated was used only for the real-time display. There were three people per side (from the field-reset crew, not the refs) watching the goals and armed with mechanical clicker-counters. They reported to the referees at the end of the match, and the head referee wrote down the scores, added information about penalties and DQs, and handed the sheet to the scorekeeper, who then entered the data into the system.
