Relay fault

Hi everyone, I met with the engineers on our team the other night to run a check-up on our robot. We quickly found that one of our window motors that uses a relay, wasn't working, even though the relay was signaling properly. After checking the connection at the window motor, we examined the relay.
The terminal that joined the relay with the wire was very loose. And oddly enough, this was the second faulty relay we had this year. Has anyone else had this problem with the innovation FIRST relays?
