HELP ME!!!

I'm filling out Forms SS-4 and 1023 for the new team. For years I've been application-challenged, and just looking at the 1023 is very deterring. Anyway, I'd like some help with a few things, wondering if anybody would like to help me out. Please. I'm begging you.
