This study is being conducted by Nana Sarpanyin, a graduate student of
the Industrial & Manufacturing Engineering & Technology Department at
Bradley University as part of the requirement for Masters in Industrial
Engineering degree. This research will help understand the different
generations of Six Sigma and its impact in organizations. At the
completion of the study, I plan to distribute results that focus on the
quantitative benefit of six sigma based on the data provided by
respondents.

I will greatly appreciate your completing this online data collection
instrument. Even if you do not know some answers, please complete this
survey. Since the validity of the results depends on obtaining a high
response rate, your participation is crucial to the success of this
study. Your completion of this data collection instrument indicates your
consent to participate in this study. Please be assured that your
