2. Have you ever been exposed to flying debris, such as dust or chips?

Yes No

3. When you use power tools and/or are exposed to debris, do you wear safety glasses?

Yes No Sometimes

4. If yes, then what percentage of the time do you wear safety glasses?

0-20% 21-40% 41-60% 61-80% 81-100%

5. Have you ever been injured as a result of not wearing safety glasses?

Yes No

6. Do you have any reasons why you do not wear safety glasses? (Please explain)
  • I don't generally wear them at work. The glasses I have there get used maybe 10 days a year due to how uncomfortable it can be to wear them 8-10 hrs a day every day. I put them on for high risk work and that's about it..
  • At robotics I wear them to set an example to the students and because it is shop policy.
  • At home I don't generally wear them unless there is high risk. I don't think I ever wore glasses before I started doing high school shop/ robotics...
