stumped - can't get servo example working

I am trying to run the servo example vi, but the digital sidecar status led is always in fast flashing mode.

My configuration:
- DB37 cable from module 4 (NI 9403) to digital sidecar.
- 12V Wago connector from PD board to digital sidecar
- no jumpers on the digital sidecar (anywhere)
- nothing else connected to the digital sidecar

When I power the system on, the BAT, 5V and 6V leds turn on. No other leds are on.

When I deploy the servo example, the status led will start to fast flash. (The flashing begins mid-way through deployment.)

Fast flash continues while the application is running.

I get the same behavior if I have a hobby servo (Futaba 3003) connected to a PWM channel (with or without the power jumper) and deploy the application. Servo doesn't respond to the dial control.

Any ideas? Thanks!

Update found this forum tread which sounds like it might be related to my problem [need to enable the robot from the driver station]:

If so - what would be easiest way to get the servo example working? Do I have a create custom Dashboard VI? I'm not using the Classmate, just my own laptop.

lol you need to put a jumper on the input next to the pwm slot the servo is connected to on the digital sidecar. They do not come with the digital sidecar

There is a little 2 pin spot where you insert the jumper. LOL I spent a full week last year trying to get servos working. My code was perfect, but it only took one question to the electrical leader to fix it

lol I had fun with the programmers last year haha I made them look it up in the manual even though I knew what the problem was haha

It looks like a tiny square of plastic, just under 1/4" on a side. The KOP checklist says you should have received four black ones along with a bunch of other things in small electrical parts bag. It fits over two pins sticking up a tenth of an inch apart on the Digital Sidecar, adjacent to the three-pin PWM connector.
