[FTC]: Recommendation for lead screw and how to attach it to a motor?

We are considering using a lead screw to extend a drawer slide. Can anyone give us recommendations on lead screw and lead nut vendors and part numbers? Also, we are unsure how to connect the lead screw to a motor. We thought about attaching a gear and axle hub to the lead screw and then connecting a gear to the motor shaft. But the inside diameter of the axle hub is only 3/16 inch, and we cannot find lead screws that small.
