Chairman's DVD Form question
While getting our presentation ready for the Championships, I noticed that the form that goes with the DVD specifically says, "TEAM CONSENT TO USE VIDEO PREPARED FOR REGIONAL CHAIRMAN’S AWARD ENTRY" (emphasis mine) Is there another form I don't know about, do we even need the form again, or do we just do it identically to the regionals anyway? I've looked around, but I can't seem to find an answer. I'm assuming we just use the same form, but I want to make sure. The official Chaiman's doc doesn't say much about the Championships.
