Last year the release forms that we had submitted at our first regional were transferred to our second regional automaticallly. We did not have to have our students/mentors sign an additional form for our 2nd regional…Is this the same this year?
Based on our experience this year, I would have a backup copy of the forms just in case. Our team participated in Boston and Arizona this year and there was some confusion about release forms (we did NOT have copies of them since we believed that the first regional covered all others). In the end, a mentor list covered us and the Arizona regional personnel that I dealt with were VERY professional about the problem. I would simply say that if you had copies or could make copies, then it might make things simpler for all parties involved.
It’s supposed to cover all events. However, if you have people who weren’t at the first event, you’ll need to have that paperwork for them.
Your best bet is to make copies, as noted, and turn in the originals. That way, if something comes up, you can say, “I turned them in at ZYX event, and here are copies to show that.”
A completed form is required for every team member and mentor who travels to a FIRST competition event. They must be turned in at the team’s INITIAL 2009 Regional event. Turning in these forms at an INITIAL event means you do not need to again bring forms to additional events, including the Championship. However, team persons who did not attend their team’s INITIAL event, and for which we do not yet have a Consent and Release form, must bring a completed form to the next event and turn in to Pit Administration during the registration process.
