Kraken x60 inconsistent motor output

We recently got some new Kraken x60s to put on our robot and we are encountering an issue with one of the Krakens. Motor output is inconsistently spinning the shaft. The lights of the Kraken are correct, but sometimes it does not spin the shaft.
What we’ve tried:
Replacing the motor
Replacing the breaker on the PDH
Creating a new robot project solely for this motor (link)

More detailed description of the problem:
Occasionally the Kraken will not spin the shaft of the motor at all. Output is inconsistent and sometimes it will spin correctly and sometimes it will not spin at all. The lights on the motor are consistent with the input we are giving it (directly bound to a controller’s axis, checked the set value on Shuffleboard)
We have 4 Krakens on the drive system and they are working just fine
